In order to tryout for a sport, a student must have a PIAA physical on file with the athletic office, have one to turn into his/her coach at the meeting or will receive one for their own doctor before the start of tryouts/practice and will be able to give this completed physical to the coach on the first day of tryouts/practice.

HIGH SCHOOL SPORTS - start date Aug. 12th
MIDDLE SCHOOL SPORTS - start date Sept 9th
ALL players must have a physical dated AFTER June 1, 2013 in order to participate
TUESDAY, JUNE 4th - PHYSICAL DAY - ALL MIDDLE SCHOOL GRADES
Since we know that the anniversary date for your child’s physical might not fall between June 1st and the start date for sports, We have made arrangements with a group of doctors to come to Haverford Middle and provide physicals for the students. Physicals will start at 2 pm in the Nurse’s office. This physical will be on the PIAA approved form and valid for all three sport seasons. The cost of a physical is $ 25.00. If you want your child to receive a physical do the following prior to FRIDAY – May 31st
E-mail Mr. Booth at school (email@example.com) and sign up for a physical. Times will be assigned in the order that the e-mails are received. You will receive a reply e-mail with the time of the physical.
On the day of the physical, your child MUST bring with him/her the following:
1. PIAA approved form – pages 1 to 6, pages 1 to 5 filled in and signed
2. Payment of $25.00 – cash or check (payable to Dr. Capista)
NO physical will be given if either the physical form or the payment is missing.
WEDNESDAY – SEPT. 4th -INFORMATIONAL MEETINGS
ALL sports will hold informational meetings after school. The meetings will run for about 20 minutes. It is important that your child plan to attend this meeting. They will meet their coach, find out the location of their locker room and practice field, the length of practice, the schedule for the first week, and what they need (forms, equipment) to be ready to start practice. Completed PIAA physical forms will be collected. The locations of the meetings are:
ALL football – Gym A/B All BOY’S soccer – room 276
All GIRLS soccer – room 155 ALL Girls Volleyball – room 251
All Field Hockey – room 279
Reminder announcements will be made at the class assemblies on Tuesday and during the morning and afternoon announcements.
